Drs. Edgar Pierluissi and Ken Covinsky (SFVAMC) and Clinical Nurse Specialist Annelie Nilsson are featured in an NPR newstory, Hospital Units Tailored To Older Patients Can Help Prevent Decline, that aired August 9, 2016. The piece highlights the risks that older adults face when they are hospitalized. Dr. Covinsky’s research establishes the centrality of functioning in the care of an older adult. Dr. Pierluissi established the first ACE unit in the state at the ZSFG. Annelie Nilsson is a clinical nurse leader on the ZSFG ACE unit.
America’s Essential Hospitals selected “Expanding the Care Continuum to Reduce Readmissions at SFGH” as an honorable mention recipient for a 2016 Gage Award. Dr. Michelle Schneidermann accepted the award on behalf of the Care Transitions Taskforce, DHM, DOM & ZSFG on June 16.
Jack Chase, MD has been awarded an early career development fellowship from the UCSF Hellman Fellowship Program. Jack intends to use the funding to support the development of care transitions communication between ZSFG inpatient services and SFHN and SFCCC primary care.
